Many, maybe most, mental illnesses show up early, especially during puberty, but there's nothing to prevent someone from losing their mind for any reason at any time. Chemical imbalances can develop, or just unexplained psychosis, at any stage of life.
Or, the most obvious choice, she developed the mental illness afterwards, and a "screening," if such a thing were even reliable, would have shown nothing. Mental illness develops in all phases of life, you're not always just born "crazy."
