This isn't a new problem. The Flickr community started experiencing problems early on - and primarily the role-players were girls (tweens/teens). It makes sense that it would be come even "easier" and more prevalent with the Instagram.

I'm pretty sure that first response would be exactly my response if I responded.

I know a number of teachers with fake names on facebook so that their students can't find them. I also have a handful of other friends that have fake names for whatever reason that I never asked. Most use a variation of their real name (intentional spelling mistakes, middle names, etc).
