Now add in the fact that he was there with five other people and there were multiple cocktails ordered (I’d guess at least some of those were before they ordered the entrees), and it seems there was probably plenty of distractions / reasons he may not have read the entire menu.

I generally ignore the items that are separated out / featured on a menu (as the Golden Tomahawk is here), because they generally tend to be the premium-priced items.
