Technically cards are only issued to players. Coaches and others are “sent off” and never shown the card as you can clearly see in the video.

Is there fault? I think there likely is. These are clearly deadly activities when things are not properly planned for or when they do not go as expected. For whatever reason, something went wrong; not likely due to poor planning given his experience. He clearly knew the risks.
