Though perhaps not strictly one-offs, giant tunnel boring machines (TBMs) are often designed specifically for one project and will often not see any work after completing said project.

It's very possible Ms. Barra knew nothing about the parts changes to the ignition cylinder. I find it unlikely that the executive director would be made aware of such a minuscule change that at the time would have seemed so unimportant.

I've been there. I think 4 or 5 cars drove around me before my uncle (who was teaching me) and I figured out what was happening. This was also the same day I stalled about 17,000 times in the grocery store parking lot with basically my whole town watching.
