Seriously!

Can anyone recommend a plug-in to generate a "Contact Us" form to my Wordpress blog?

I have recently used [www.jotform.com] which i found to be a useful form building tool. However there was no integration to spreadsheet/database, only submit to an email address.

Does anyone know of a web app that can be used to track the lifecycle of a sales opportunities?
