I'm not sure they've clarified what they mean by 'observed' either. The assumption seems to be 'observed by a cognizant being', as otherwise one buzzing housefly could take on an entire principality of angels.

The bit with the 'etched in stone' seemed spot-on, and segued with what I've thought for a while: the reason the Doctor always seems to unknowingly land in times and places whose dangers he could have easily avoided is that he goes out of his way to know only the barest of details.
