I'm assuming that he's the killer all along, & just blamed his real sister for killing his parents (& I assume he killed her, too), but the guilt he feels for killing his sister makes him keep "her" "alive".  This is also more in keeping with the Psycho source material: Norman killed his mother when she tried to date
