I assumed she wasn't there, and we didn't see her die, because when Dennis bothers to think about anyone but himself he never thinks about Dee. To him she's not part of The Gang.

I would suggest the other rule of Martin is many your favorite characters are going to die.  I am so happy that Tyrion's made it this far.  I'm assuming there's a resurrection for John coming, but I'm sad to think of him all burned out and creepy like Dondarrion.
