I'd say that Juliet either kept Kate's name off the list, or simply took her sweet time 'saving' Kate from weasel-guy (ie there weren't enough places on the manifest for Kate, necessitating someone coming to her aid). Either way, Juliet put Kate in her place, much as LaFleur did with Jack.

Sky1 (UK) showed a couple of specials, nice behind-the-scenes stuff, just before s5 kicked off. They were filming eps 9&10, and Elizabeth Mitchell's nipples were standouts.
