I think he used the dead body of Moriarty with the Sherlock mask that Moriarty apparently had (since the little girl screamed at the sight of Sherlock), and Dr. Watson was injected with a tranquilizer when the bicyclist ran into him, thus hallucinating what he (Watson) expected to see.
Either that, or I am a huge nerd…
