Plot holes: you forgot to mention the prudish morality of watching the shower tapes. I'd assume you would watch the Tabula Rasa's all the time, ala Clonus or The Prisoner, and have a whole team of people that looked for the slightest signs. Especially after the Alpha Incident.
I think Topher—and the rest, probably—are all Dolls themselves. Therefore, anyone can be Topher simply by plugging in the right personality.
