Madame Gao being from K'un-Lun and a member of the Hand are not mutually exclusive. I wouldn't be surprised either if the Hand's origins lie in K'un-Lun. That their leaders are outcasts from there.
I would be very surprised if they weren't the Big Bad. They're a large enough and powerful enough group that I could see the four Defenders having to band together to stop them. And because the first season of Daredevil teased Iron Fist's connections to the Hand through the Steel Serpent emblem.
My guess is that'll be explained in the Defenders mini-series. The Hand resurrect Elektra, and then Matt volunteers to become Black Sky at the end to "save" her.
