There was a moment early in the series where Walt and Jesse are talking in the RV, and Jesse clearly calls him "Walt" while talking to him. I can't remember which episode but I'm 100% certain it happens.

I have to agree with you on the pacing thing. One thing I didn't like about The Crimson Horror, while it was necessary, they took a bit too long to actually get to The Doctor. I have to give this at least a B+. The burden on this finale was that it had to be written to be more of a lead-in into the anniversary episode, so questions had to come up that were left unanswered, but for what it was it was handled amazingly.
