I have worked for fancy pants doctors on the Upper East Side, and it's not uncommon for these people to work one day a week at a clinic and/or have a sliding scale of fees

Recently they seem to be shuffling the schedule a couple times a week. For a while it was all Ghost Whisperer and Criminal Minds; for a week or so it was all Monk and Psych; now it's all George Lopez and Nutty Professor Movies.
