Cliffhanger?
I'm not sure it's a cliffhanger, in the sense that I think it would be a brutally powerful way to end the who. Imagine if this was a series finale, wouldn't that be an incredible way to go out, with the main characters having essentially broken bad?

But I feel like often when authors (like, of books) describe their process it involves winging it and seeing where the story takes them. So, novelistic tv and all that.

I disagree. I think the scene was there to give the audience a glimmer of sympathy for Sonny - to tell us that he wasn't lying about the rescue boat, making us think perhaps he would turn out to be decent after all, before dashing it all on the crack whore rocks.
