I actually managed it in one playthrough, but only because I was
actively trying to see what would happen if I leave my scythe (pretty
much the main item) in one level of the game and continue on to the next
one. It is gone and you cannot continue!

I'm only eight episodes in, but so far I think that season two is a modest step down from season one, though I may only think that because Crazy Eyes never urinates on the floor while maintaining eye contact this season.
