It is definitely a blurry line. I tend to toss albums that play with characters and plots but also include songs that aren't part of any sort of a story, but have similar themes, in the "concept album" pile as well. I think a rock opera is distinct in that it tells a more or less linear story.

Concept album? The Dark Side of the Moon, but since that's practically cheating, otherwise probably Gentlemen. Duke, Misplaced Childhood, and Pyramid are damn strong contenders.

I was going to get all offended because I think American Idiot is a phenomenal album that tapped a main vein of the American zeitgeist during the Bush era, but then I remembered Jon Daly is a comedian and it's probably not worth getting too wrapped around the axle.
