I can sort of buy it as in character for O'Brien  - she seems to either a) treat people with devotion, like S2 and 3 Cora, S1 Thomas, Alfred, b) mild contempt, which would be most of her coworkers or c) attempt to ruin their lives for a disproportionately minor reason  - Bates, Cora when she thought she was being

I do like that we've had the follow-through on what happened to her (I didn't warm to her much last season, but she's grown on me a lot this time round). In fairness, I guess, this is happening over the course of 9 years - the audience is just getting the two times he's misread the situation (in this case, pretty badly, and yeah, I find the overtly hands-on approach with Jimmy foolish and really quite uncomfortable.)
