If I'm not mistaken, the episode titles for the first 3 seasons weren't even really intended to make it to the fans generally speaking - and if they were, they'd be less awful. I wouldn't read too much into them.
I have a shaky memory of them believing for a few episodes that Scorpius likely had been killed here, but seeing it now they clearly have like barely one episode before he shows up again and they never even assume he's dead.
