Of Hannah, Arlene, and Deb it seems to me the one who is the LEAST likely to have done it is Hannah.  Serial killers don't suddenly start using pharmeceuticals after spending their whole lives poisoning people with organic plants that don't show up in most poison screens.  Xanax doesn't fit Hannah's M.O..

Looking at the narrative from Otto's perspective, it would've made more sense for him to just kill Tara, not the nurse.  If he wants SAMCRO to bleed, Tara being dead hurts Jax a lot more than Tara getting brought up on charges (plus it will make the rest of us a lot happier).
