I interpreted that as 'they have a slightly naive view on how long it will take to be able to afford to be back in a nice bigger place' rather than 'Michael is going to die soon'.

Personally I don't care that these people are part of all the big tech products/revolutions because it's interesting to explore them through a lens of fiction. I've never completely understood the setup but I believe it's a practice tied to a hospital (the one she later visited with Leo) and all the nurses from the hospital (and thus this practice and others) were striking. Maybe they tour all the practices as a group.
She also has her fertility clinic which seems to be doing well and that's an extra source of income.
