Because they’re an extremely niche market and don’t end up manufacturing many. Low demand, low supply, high production cost per unit.

There are other reasons for it, too. Chaining SOS battles is supposed to spawn Pokemon with better IVs and hidden abilities. It’s also the only way to get certain Pokemon to appear. And it’s nice if you just want to farm a lot of the same thing.

I imagine it works very well in areas with dense pokestop placement, but players in rural areas (or areas that otherwise had low Ingress numbers submitting portals) are getting messed over pretty bad.

Nope, Ethan nailed it. You don’t need a new town for the new update, but I imagine a lot of towns are going to be abandoned again now that Pokémon is out.

Again, though, the JUSTIN BAILEY code isn’t something inserted by the developer. It’s just a legit combination of readable letters (and blank spaces) that fits a working checksum. If you’re patient enough you can come up with more codes just like it using software to test the checksum.
Err...there’s nothing magic about JUSTIN BAILEY. It abides by the game’s checksum system and isn’t even the “ultimate” Metroid code, since you have to go track down the ice beam.
