Kelsey-Hayes AW4 and EBC4 brake systems on mosy early 90’s-mid 90s GM trucks. Crappy detection and malfunctioning sensors.... over 1.1 million cars with it, and as far as I can tell, no real resolution aside from a “voluntary recall”.

I think he means the 3 piece spark plugs (3V) that have a tendency to build up carbon on the tip. Once that carbon collects, it keeps the plug from coming out, and upon attemping to the tip breaks off in the combustion chamber. There’s even special tools for it.
