No - if the weight isn't presenting any medical problems (yes, it is possible to be "overweight" but have no medical problems) it is not appropriate to bring it up.

It is unbelievably dodgy how now of this stuff has fixed prices. Basically, medical centres negotiate with each insurance company how much to charge for something and then just charge the uninsured whatever the fuck they want. 'Oh, you had a wart frozen off and it took 5 minutes? That'll be $3,000'.
I love that it's supposed to be a radical idea that doctors hold biases just as much as everyone else in the world. They're people too, and people stupidly judge and shame others.
