If you look solely at teething problems and ignore all dubious design decisions, there really hasn't been any launch problems for high-profile MMOs since Aion's long queue times. If you want to talk actual buggyness and unplayability, WoW is the last one I can remember.
The majority of more recent MMOs have had pretty solid launches. I definitely do think that the industry has learned their lesson in that regard.
Yeah, I thought that was pretty stupid as well. You can fit a silencer to just about everything in the game, but the ONE thing you'd likely use for stealthy lethal takedowns cannot actually be fitted with one.
