I thought it varied by business since I know underage gas stations here (Iowa) can sell booze and cigarettes to people of age, but at Walmart and chain Midwestern grocery stores they usually have to call someone over to sign in. I don’t know why it’s the rule but lots of alcohol purchase rules seem pretty arbitrary

I don’t think they mean this article criticizes people with special dietary needs, but that the type of zealots described in the article do. And yes, those people absolutely do recommend people follow their diet with no medical knowledge.
