Early racing Ferrari's like the first few 250TR's and also the Maserati 250F have the accelerator pedal in the middle, the break on the right and the clutch on the left. A few have been crashed because of this.

That's a classic safety feature that was present in pretty much all manual transmissions from the early 90's backwards. Both pushing down or pulling up on the lever releases the reverse lockout. Anyone who's had to drive manual cars for long enough/in thrid world countries knows about this. I actually can't believe so
