Speaking as the guy who translated both Lain and SAC, you have it backwards. I did the sub translation first, and the dub script was made from that. That’s how it’s always done. I guess it’s possible that some compilation rereleases that are on store shelves these days repurpose the closed captioning track from the TV

It’s a thing. All of these are the result of people not reading books. They’ve never seen the words in print (I mean actual edited, published material, not internet posts), so they just repeat what they think they heard. It’s the telephone game in action.
