With a modern cable-operated shift mechism, the gearstick can be pretty much anywhere. All you need is enough space for the throw of the levers so they can pull the cable far enough. Routing the cable to the gearbox is not so hard.

I’m more of a Sugar Rush guy myself, but in real life I’d happily drive any of those cars around a major conurbation.

I usually take my hand off, yes. In the unusual case that my hand is on the shifter for more than a couple of seconds I don’t rest any weight on it.

Why would you keep your hand on the shifter? That’s what the steering wheel is for. Resting your hand on the shifter is not good for the transmission.
