As I was reading the book I just started making any character who was "default white" not white in my head. All of a sudden, WOW, this book is so diverse!

That was my first thought as well. Anyone who is trans (male or female, really) could have a reason to go to the gynecologist. I know there is an exception mentioned, but it could still go very very wrong.
