Bluray discs have a scratch resistant coating, and any disc drive worth their salt won't have abrasive components that would contact the disc easily.

Ironically, if they used a Monster Cable, the PS4 HDMI would probably be way more worse for wear. Monster Cables are loathed for many things, but one in particular is damaging whatever you plug them into.

My guess is where ever they contract their HDMI parts from, probably rushed the job and sent them a batch with some faulty ones. Or maybe during quality testing, perhaps workers were moving at a clip, and get aggressive with jamming in an HDMI, testing the output, then put it back on the line for the next one.
