My understand was a combination of what you laid out caused by a lack of development time, despite coming to the current engine formula ONE YEAR after all other manufacturers adopted the 1.6l turbo/hybrid layout.

Actually, the cargo holds of most all airliners are pressurized. The floors of the planes are not designed to handle the pressure differential the way the round cross section hull is.
