I think we'd need more details for sure before we can make judgements on this, but on the face of it, it sounds horrific. As someone who works for social services in another region of the UK I always feel so shocked and ashamed when I hear of massive failings.

Why wonder? If she could, she would have. Since she didn't, she couldn't. As for the specifics of why, it can't have been pleasant: maybe the kid was pinned down by wreckage, maybe impaled, who knows? I wouldn't question the mother's level of care here.
