My Spider has just over 100k miles and so far I have had zero issues, though to be fair I've only had the car two years. What really does it for me is the sound; it still sends shivers up my spine every time I start it up.
As a vintage Fiat owner I can attest to the awesomeness of the Twin Cam, it was incredibly advanced for its time. They tend to heat up quite a bit, but as long as you're good to it, it will be good to you.
