Well, the game does say there are Great Ones that aren’t totally against humans, which reside in dreams. There are also “formless” Great Ones, so it’s possible one could be inhabiting the Doll as a vessel.

For making them “mainstream,” I’d give the credit to Game Center CX, and I’d say Giant Bomb’s quicklook format has had a bit of an impact too.
