It happens all the time. The hospital/doctor performs the treatment anyway and usually the hospital is stuck writing off the loss.

This is true, but the other part of insurance is paying premiums based on your risk. If you have had two or three speeding tickets, you are going to pay a higher rate than someone who has had none. If you decide to own a dynamite factory, you’ll pay higher premiums than if you own a bakery.
