That's probably a matter of anatomy. I can't speak for anyone else, but everything I have is kind of tucked up and away, so going commando with pants (no VPL ever!) isn't at all uncomfortable.

I never have any issues with the crotch seem mangling my labia, you should check into your pants choices. As well, it is pretty damn easy to keep your skirt from flying over your head, even without underwear on.
