Basically, you close the YouTube account yourself, and send a support ticket asking for your account to be reinstated. (Hopefully you still have access to the e-mail that created the YT account)

It's annoying with YouTube because Google will flat-out not let you unlink YouTube accounts from Google Accounts, meaning that if you link your YouTube account to the wrong Google Account, you're screwed.

It's not free until they allow Google+ access for freaking Google Apps accounts.
