You're not wrong. A 99.8% sensitivity means a test appropriately identifies 998/1000 people with the disease. But there are other factors to consider.

I don't think "instant" is a relevant standard here. The word does not appear in my reply. But there actually are very rapid (<10 minutes) HIV tests that are virtually perfect (95% confidence interval for sensitivity, 98.8-100% in this study, 99.8-100% based on these CDC slides).
I think vCJD is a bit different, though. I'm not aware of any viable test, and symptomatic manifestation can be delayed (though 34 years is probably unusually long).
