It’s all been revealed in Civil War 2. The “premonition” of Miles holding a dead Steve Rogers. Steve has to die, which will then “reset” his brain, and when he comes back to life, he’s good again.

I think it is more a visual representation of how the red skull is in his head because he basically brain washed Steve this way and still has Xavier’s brain sewed up into his dome.
