HSPA+ (21mbps) is labelled as '4G' which is what the iPhone will have. And it says '4G Data speeds' doesn't actually says it's 4G (as in LTE)
If Cincinnati Bell made this page with true information from Apple there are two things here that point to a 64GB iPhone 5.
Where do you live that 'SIM only' accounts are cheaper? In Canada (and I believe US), every company will charge you the exact same amount if you buy the phone from them at a discount or if you bring in your own phone you bought off of eBay. (I work at Rogers so I know this to be true in Canada at the least)

I don't necessarily think they would have a new icon file for it. If it's the exact same casing then there is really no point. As well, it shows the CDMA icon which would make sense as the CDMA iPhone has the updated antenna design, which the iPhone 4S would presumably have as well if it exists.

It's incorrect. iPhone 3G only had 3.6mbps download chip and 512kbps upload. iPhone 3GS moved the download to 7.2 and kept the upload at 512mbps. iPhone 4 kept 7.2 down and moved upload to 5.?? to support FaceTime

It needs to have a magic word feature. As well as true voice recognition (distinguishes my voice from the stranger beside me). That way I can say 'iPhone send John a text, tell him I'll be late' and it recognizes my voice and does it. That would be ideal, but is not great for battery life
