Yeah the war assets have 3 thresholds which either give you everyone dies, big ben dies, earth is left untouched after the explosion.

Pretty much every sidequest item you miss you can buy at the spectre terminal some quests need to be completed before doing a certain 'Priority' mission otherwise they become uncompletable.

In response to the first part neither did Mass Effects 3's ending.

Red Dead Redemption, Metal Gear Solid 4 (And the others for that matter), BioShock, Portal etc etc all these games have unforgettable great endings so it is possible. Take note BioWare.
