Lynch has a different work ethic than most film makers. He doesn't want to hit you over the head, he wants you to pay attention. If Lynch made a sitcom, pretty safe bet he wouldn't use canned laughter to let you know something funny just happened.

I think somewhere in this episode something happened that wasn't supposed to happen and it changed time, caused a ripple in the fabric of time that manifested as the change at the diner at the end.
