A restaurant that aims to be “high-end” wouldn’t offer a burger in the first place.

And while declining Part B carries risk, the cost of that risk sure does seem disproportional to the cost, and also wildly unpredictable and surprising. I routinely charter Honda Jets and fly coast to coast for $50k.  This charge from the ambulatory company I would classify as predatory.

While several responses are “they opted out of part B, so live with the consequences of their actions”.... Stop it, this woman is over 65 and probably on a fixed income where they COULDN’T afford it. The sheer fact that there is a $71K difference between insured and uninsured is proof that they are price gouging.

The larger issue is that vendors charge insurance companies fees that are far less than the fees they charge uninsured consumers - this should be changed and fees should all be the same regardless of insurance coverage - so if the air ambulance company would have accepted 10,000 from insurance they should do the same

I would agree IF the flight costs were reasonable. $81K for a 79 mile flight is not reasonable. Knowing “air ambulance bills were high” is not the same as getting a $81 K bill, especially when if she had been covered by Medicare the air ambulance company would only have been allowed to charge $10K. That is a $71K
