Sure, I completely agree that human-error/laziness is a huge factor, but it is also a factor in thoroughly washing and safely preparing produce as well. That is why I said in my original comment that the best advice is to know where your food is coming from, i.e. maybe skip fast-service restaurants altogether.

Sure, but cooking times/procedures are fairly prescribed, especially at fast food places to prevent exactly that. Also it’s only my assertion because I’ve read about it several times...

Meat is actually the safer choice at most these places because it’s grilled/fried/microwaved etc. Raw vegetables are usually the culprits in major food-bourne illness outbreaks because they aren’t cooked and improperly washed/stored.
