Actually, no. That is up to the seller (publisher or dev) as to how they want to integrate DRM schemes within Steam. If you really wanted to, you could have a DRM-free game released on Steam.

Yes, it would be hard. The netcode would need to be reworked and beyond that, it would also need to be QA'd and possibly patched after release. This adds a lot of overhead. It's easiest for them to release via the 360 port.
It's a case-by-case thing. Thing is, MS doesn't have single GfWL client. For example, GfWL for GTA4 isn't the same version of GfWL used for Dead Rising 2. They can update GfWL versions, but most devs don't do that after a certain point. It simply isn't worth it.
I'm no fan of GfWL, but I understand why they're using it. The porting issue is one of the reasons why I'd love to see Steam on PS3 more. It would make porting to a Steam PC release that much easier for console-centric publishers, like Capcom.
