In a recent update, he added the Phantasy Star III/DQV option - if you successfully beat the game, you can have a child with one of your surviving boytoys or girltoys and that child becomes the new player (and the game gets harder). Probably not with Iny.

The last two (the skeletons and the giant demon) looks definitively off in Bloodborne, they look like they belong in Dark Souls.
