I’m pretty sure that the most lowly criminals of Gotham were exposed to the Cloudburst Fear Toxin so they are still suffering from after effects from it. As far as I know, Ivy’s plants only removed the Fear Toxin Cloud the engulfed Gotham that time but didn’t exactly cure the effects.
