Lexus tech here. Not trying to badmouth the technician that worked on your dads car, but people make mistakes. I’m positive the technician caused the oxygen sensor to fail. If you check the invoice, I’m betting it was Bank 2 Sensor 2, and when you spin off the oil filter on the 3MZ-FE engine, it will soak that sensor
